The short answer
- Dialysis is a knockout question on virtually every fully underwritten and simplified issue application.
- Guaranteed issue remains: no health questions, up to about 25,000 dollars, two year wait on natural death.
- Check group life through work first - it often needs no health questions at all.
- A successful kidney transplant reopens underwritten cover after a waiting period.
Where you stand
Dialysis changes the route, not the answer
Being on dialysis does not leave you uninsurable. It moves you away from underwritten policies toward cover that asks no health questions - smaller, dearer, but genuinely available.
Check my options →What you can still get
- Any policy you already have - keep it in force. A policy issued before dialysis cannot be re-underwritten, and it is almost always better than anything you could buy now.
- Group life through an employer, union or association - often available up to a set amount without health questions, particularly when you first become eligible.
- Guaranteed issue whole life - no health questions, usually 2,000 to 25,000 dollars, with a two year waiting period on natural death.
- Accidental death cover - cheap and asks nothing about health, but pays only for accidents, so it is no substitute for life cover.
- Underwritten cover after a transplant - see below.
What each situation typically means
| Your situation | Realistic route |
|---|---|
| On hemodialysis or peritoneal dialysis | Guaranteed issue, or group cover |
| End-stage kidney disease, awaiting transplant | Guaranteed issue now, reassess after transplant |
| Kidney transplant under 2 years ago | Postpone, or guaranteed issue |
| Kidney transplant 2+ years, stable function | Rated, fully underwritten possible |
| Off dialysis after an acute kidney injury | Underwritten on current kidney function |
Understand the two year wait
Guaranteed issue policies pay the full benefit for accidental death from day one, but if death from natural causes happens in the first two years, they typically refund the premiums paid, usually with interest of around 10 percent. Ask for the exact terms in writing before you buy.
Is guaranteed issue worth it?
It depends on what the money is for. If the aim is to make sure a funeral is paid for, a modest guaranteed issue policy can do exactly that once the waiting period has passed. Price a funeral first - our funeral cost calculator turns three quotes into a cover target - and buy that amount, not more.
Compare the total premiums you would pay over a few years with the benefit. For some people, setting the same money aside in a savings account or a payable-on-death account is the better choice, because it is available immediately and has no waiting period. A good agent will tell you if that is your situation.
After a kidney transplant
A transplant changes your position substantially. Kidney recipients have the most routes of any transplant group, and after a waiting period - often two years or more of stable function on anti-rejection therapy - a number of carriers will write cover at a rating. See life insurance after an organ transplant and, for earlier stages of kidney disease, life insurance with kidney disease.
The order to do this in
- Keep every existing policy in force - do not let one lapse
- Check group life through your work, union or associations
- Price what you actually need before buying a no-questions policy
- Compare guaranteed issue offers on price and waiting-period terms, not just the benefit
Common questions
Can you get life insurance on dialysis?+
Not through fully underwritten or simplified issue policies, where dialysis is a knockout question. Guaranteed issue remains available - it asks no health questions, but cover is limited to around 25,000 dollars with a two year waiting period on natural death.
What happens if I die in the first two years of a guaranteed issue policy?+
If death is from natural causes, the policy typically refunds the premiums paid, usually plus interest of around 10 percent, rather than paying the full benefit. Accidental death is normally covered in full from day one.
Does life insurance through work cover dialysis patients?+
Often, yes. Group life through an employer is commonly offered up to a set amount with no health questions, especially when you first join or during open enrollment. Amounts above that usually need medical evidence. Check your benefits before buying anything else.
Can I get better cover after a kidney transplant?+
Yes. After a waiting period, often two years or more of stable function on anti-rejection therapy, a number of carriers will write kidney transplant recipients at a rating, which can mean far more cover for the money than guaranteed issue.
